TI-RADS calculator
ACR TI-RADS for thyroid nodules. Score each nodule and the findings text for your report builds underneath as you go, ready to copy. It all happens in this page: nothing you enter is sent anywhere. It stays in this browser tab, so a reload loses nothing, until you clear it, close the tab, or leave it unchanged for 9 hours.

How it scores
- Points and categories are ACR TI-RADS (Tessler et al., J Am Coll Radiol 2017): composition, echogenicity, shape, margin and echogenic foci add up to TR1 to TR5; the size then decides between no action, follow-up and FNA, at the ACR's thresholds of 5, 10, 15 and 25 mm. The number beside each choice is the points it adds, and the colour of a choice is its points: green 0, amber 1, orange 2, purple 3.
- The small picture beside a choice opens an ultrasound example of it.
- Sizes are in millimetres; the largest of the three decides. With the cursor in a size box, the mouse wheel or the Up and Down arrows step it by 1 mm. A spongiform nodule scores 0 whatever else is chosen.
- Colloid cyst is TR2 outright and needs only a side and a size. Skip: subcentimeter nodule gives a nodule no line of its own; skipped nodules, and the not imaged box, share one line at the end of the report.
- Choosing Cystic sets the echogenicity to anechoic; you can change it after. Under Usual nodules, Solid hypoechoic and Solid isoechoic set solid, smooth margins, shape not assessed, and the echogenicity they name; the button stays lit while the nodule matches it.
- Up to four nodules are written out in full, in the order you added them; with more, the four that need the most (FNA, then follow-up, then the largest) are written and the rest share one line. The list below the report names every nodule left out and why, and warns when one of them still meets criteria.
- Each line says what the nodule meets the criteria for; it is not a recommendation in your voice. Like everything on this site, the calculator is not cleared or approved by any regulator and has not been validated: you read the nodule, and you check every line before it goes in a report. It is provided as is, under the site's terms of use.
